Responsibilities

• Provide clinical services within the scope of practice, as defined by state laws, in accordance with the plan of care, and in coordination with the healthcare team.
• Conduct initial and/or comprehensive nursing evaluation visits, determine the primary focus of care, develop the plan of care within state-specific guidelines, and ensure accurate, complete, and timely documentation.
• Regularly evaluate and re-evaluate the patient's nursing needs, at least every 60 days or as conditions change.
• Perform comprehensive assessments at designated times and develop appropriate plans of care in collaboration with physicians.
• Ensure patients meet home health eligibility and medical necessity guidelines as defined by the payer source.
• Initiate, develop, implement, and revise the plan of care collaboratively with physicians and other healthcare professionals involved in the patient's care.

Education and Experience

License Requirements

• Current RN licensure in the state of practice
• Current CPR certification
• Valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
