Job summary

Liberty Home Care in Whiteville, North Carolina, is seeking an experienced Home Care Registered Nurse (RN) to join our team. You will actively participate in interdisciplinary care planning and deliver nursing services according to the plan of care established by the Patient Care Coordinator.

Our goal is to support recovery at home with independence and high-quality care. The ideal candidate will hold an RN license in North Carolina, have 1+ year of clinical nursing experience, and possess CPR

Qualifications

Completed nursing education from an approved school.
Current RN license in the state where the branch is located.
At least 1 year of clinical nursing experience in an acute or long-term setting; home health preferred.
Knowledge of medical-surgical nursing principles and patient care.
CPR certification.

Responsibilities

Participates in the interdisciplinary care planning process.
Provides nursing services per the care plan established by the Patient Care Coordinator.
Promotes favorable outcomes through collaborative practice and efficient use of resources.
Documents care accurately to support coordination, reimbursement, and quality improvement.
Maintains professional relationships and accountability.
