Job summary

Benedictine Living Community-Duluth is seeking a Full-Time Home Health RN to provide specialized nursing care in residents’ homes in the Duluth area. The role involves assessing patients, planning and coordinating care, and ensuring safety and high-quality outcomes under the nursing process.

The RN will educate families and caregivers, collaborate with the health team, and document care plans while upholding Benedictine’s Mission and Core Values of Hospitality and Respect.

Qualifications

• Current state licensure as a Registered Nurse (RN) in good standing.
• BLS certification.
• Clinical experience preferred.

Responsibilities

• Perform assessments on patients/residents and reassess per policy.
• Develop and implement care plans tailored to each patient’s needs and evaluate progress.
• Assess and supervise LPN and CNA/NAR competencies.
• Educate patients, families and caregivers about care plans and medications.
• Document according to standards and policies.
• Coordinate discharge planning and collaborate with the care team.
• Support Benedictine Mission and Core Values in daily activities.
