Job summary

T J Samson Community Hospital is seeking a Home Health Registered Nurse to administer skilled nursing services, document care, and supervise aides and LPNs. The RN will also provide backup nursing for Hospice and participate in case management for home-based services.

Requirements include an active Kentucky RN license, current BLS certification, and two years' nursing experience. The role entails patient education, care planning, and collaboration with the interdisciplinary team.

Qualifications

• Active RN license and eligible to practice in Kentucky.
• Current BLS certification.
• Two years' experience as a licensed nurse.

Responsibilities

• Perform patient assessments including age-appropriate OASIS information.
• Provide direct nursing services as ordered by physician for pediatric, adult, and geriatric patients.
• Document delivery of patient care and visit records.
• Supervise home health and hospice aides and licensed practical nurses.
• Provide case management for Home and Community Based Services.
• Complete forms and obtain prior approval for Home & Community Based Services and EPSDT.
• Take call as assigned by Home Health/Hospice Coordinator.
• Educate patient and caregiver on disease management and consult with physician as needed.
• Review and update patient medications timely.
• Explain hospice philosophy, purpose, scope and role to patients/families.
• Prepare and implement treatment plans with patient and family and interdisciplinary team.
• Order medications, supplies, and equipment as needed.
• Instruct caregivers in proper use of medication, supplies, and equipment.
• Function as liaison between patient/family and other care providers.
