Job summary

Cambridge Caretenders Home Health, part of LHC Group under the Optum family, seeks a Registered Nurse in Home Health to provide and direct nursing care in patients' homes as prescribed by physicians and in compliance with applicable laws and agency policies.

You will coordinate the total plan of care with other professionals, supervise home health aides, complete comprehensive assessments, document per policy, and ensure quality care while communicating with patients, families, and physicians.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License or access to dependable transportation.

Responsibilities

• Direct provisions of nursing care to patients in their homes as prescribed by the physician.
• Coordinate total plan of care with other health care professionals.
• Supervise home health aides and licensed staff; provide instruction and assign tasks.
• Complete comprehensive assessments (OASIS) and documentation per policy and payer requirements.
• Engage in on-call and weekend rotation as needed to meet patient needs.
