Job summary

Qualifications

• Associate degree or nursing diploma from an accredited school.
• At least 1 year of RN experience in an institutional setting.
• CPR certification, California RN license, valid CA driver’s license, current automobile insurance.

Responsibilities

• Provide skilled nursing care to patients in their homes under physician orders.
• Develop and coordinate the care plan with the physician and interdisciplinary team.
• Evaluate patient and home environment; report changes in condition.
• Maintain accurate documentation in the patient’s medical record.
• Direct and supervise home health aides and ensure safety/infection control standards.
