Job summary

Advocate Health is seeking a Full-time Registered Nurse – Home Health in the Chicago area. The RN will work 32 hours per week with Monday–Friday hours, plus weekend rotation every 4th weekend and two holidays per year. On-call after hours coverage is rotated among the nursing team.

A sign-on bonus up to $3,000 is available for eligible internal and external candidates. Responsibilities include building therapeutic relationships, coordinating care across settings, educating patients and families,

Qualifications

• Graduate from an accredited school of nursing.
• One year of medical-surgical experience.
• Current Illinois RN license and CPR certification are required.

Responsibilities

• Build and maintain therapeutic nurse-patient relationships across the care continuum.
• Assess and coordinate care with patients and families to achieve outcomes.
• Provide education and support to patients and families to promote self-management.
• Collaborate with the nursing team to ensure quality, timely, and compliant care.
