Job summary

OSF HealthCare is seeking a Registered Nurse for Home Care in Peoria, IL. The RN Case Manager will provide patient-centered home healthcare across all ages, coordinating inter-disciplinary care and ensuring appropriate visit patterns and outcomes.

This PRN/hourly role requires current RN licensure in Illinois or Michigan, BLS, and a valid driver's license. Prior EMR and MS Office familiarity is preferred; the position offers flexible, on-call scheduling to support patient care needs.

Qualifications

• Current licensure by the State Board of Nursing in Illinois or Michigan.
• Current American Heart Association HealthCare Provider BLS certificate is required prior to start date.
• Experience: maintains own transportation and auto insurance and abides by all laws while on duty.
• Maintains a valid driver's license.
• Experience with Electronic Medical Record and Microsoft applications is preferred.

Responsibilities

• Coordinate inter-disciplinary care for home health patients.
• Provide patient-centered care and manage episode care to ensure positive patient outcomes.
