CenterWell Home Health in Winston-Salem, NC is seeking a Home Health Registered Nurse to deliver one-on-one skilled nursing care in patients' homes, under the Clinical Manager. You will administer medications, perform wound care, and assess recovery progress.

You will develop individualized care plans with physicians, educate patients and families, coordinate with the care team, monitor changes, and oversee Home Health Aides, ensuring accurate documentation.

Qualifications

• Active RN license and CPR certification.
• Experience in Med-Surg/ICU/ER or acute care.
• Ability to collaborate with physicians and clinicians.
• Reliable transportation to in-home visits.

Responsabilités

• Provide skilled nursing care to patients in their homes, including medications administration, wound care, and patient assessments.
• Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals.
• Monitor patients’ conditions and report changes.
• Educate patients and families on disease management, medications, and treatment options.
• Maintain accurate records of patient care and coordinate with other healthcare professionals.
• Report patient care progress to patient's physician and Clinical Manager.
• Oversee the work of Home Health Aides and ensure documentation is completed.
