Qualifications

Responsibilities

• Provide admission, case management, and follow-up skilled nursing visits for home health patients.
• Administer ongoing care and case management for each patient.
• Confer with physicians on treatment plans and assessments.
• Coordinate appropriate care across various healthcare personnel.
• Monitor and supervise the work of Home Health Aides.
