Job summary

Otterbein SeniorLife is seeking a Registered Nurse – Case Manager to provide comprehensive home health services across Lebanon, Middletown, Springboro, Dayton, and nearby areas. The role requires 1 year of clinical nursing experience, an Ohio RN license, and CPR certification.

The position includes on-site visits, care planning, medication administration, and coordination with physicians to ensure high-quality patient-centered care. Valid driver’s license and reliable vehicle are required.

Qualifications

• Bachelor's degree in nursing preferred or equivalent.
• Valid OhioRN license and CPR certification are required.
• Minimum 1 year of clinical nursing experience; home health or hospice experience preferred.
• Must have reliable transportation and clean driving record per Ohio law.

Responsibilities

• Perform initial patient assessment and care planning.
• Develop and revise care plans with patient and family involvement.
• Administer medications and treatments as prescribed by physician.
• Coordinate with physicians and community health partners for ongoing care.
• Provide patient and family education on health management and discharge planning.
• Document care plans and communications with care team and physicians.
