Job summary

Amedisys, Inc. is seeking a Registered Nurse Case Manager for the Baylor Shift on weekends and one weekday. You will assess patients, develop and implement care plans, and collaborate with the care team to support home-based healing.

Qualified candidates have 1+ year RN experience, an active RN license in the assigned states, CPR certification, and a valid driver's license with insurance. Amedisys emphasizes growth, flexibility, and compassionate home health delivery.

Qualifications

• 1+ year of clinical RN experience.
• Active RN license for the state(s) assigned.
• Current CPR certification.
• Valid driver’s license with reliable transportation and liability insurance.

Responsibilities

• Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
• Make referrals to other disciplines as needed and document rationale.
• Promote patient health and independence through teaching and rehabilitative measures.
• Supervise LPNs and HHAs.
• Complete documentation timely and accurately.
• Regularly communicate patient progress to the clinical manager and care team.
• Plan and provide staff education.
• Perform on-call duties as assigned.
• Participate in clinical development and continuing education.
