Job summary

PruittHealth is seeking an experienced Registered Nurse (RN) to join our Home Health team in York County. This full‑time role offers autonomy, meaningful one‑on‑one patient care, and a balanced schedule in patients’ homes.

As a Home Health RN Case Manager, you will perform skilled nursing visits, start‑of‑care assessments, care planning, and collaborate with the care team. A valid RN license and driver’s license are required, with opportunity for growth and ongoing clinical support.

Qualifications

• Active, unrestricted RN license
• Valid driver’s license and reliable transportation
• CPR certification
• 1+ year of RN experience (home health or acute care preferred)

Responsibilities

• Provide skilled nursing visits in patients' homes
• Complete assessments, start-of-care visits, and care plans
• Collaborate with interdisciplinary care team
• Supervise LPNs and Home Health Aides
• Document care using an iPad with AI assist (OASIS experience a plus)
• Educate patients and families to promote independence
• Ensure continuity, quality, and safety of patient care
