Job summary

Nightingale Home Healthcare Of Minnesota Inc is expanding its team of full-time Registered Nurses to provide skilled in-home care in the Minnetonka area. This PAY PER VISIT role offers a starting rate around $60 per visit and the potential to earn up to $120k annually based on patient census and visits.

Responsibilities include obtaining patient history, performing physical assessments, coordinating with LPNs and aides, and implementing the care plan under the physician’s orders.

Qualifications

• Licensed RN in MN required.
• Minimum 1 year medical/surgical or Home Health (OASIS) experience preferred.
• Current TB results within last 12 months.
• Current CPR Certification required.
• Valid driver’s license and liability insurance.

Responsibilities

• Obtain medical history from patient/family.
• Conduct brief physical examination and assess vital signs.
• Evaluate patient/home situation to determine required health teaching.
• Coordinate care with LPN, Home Health Aide, and Personal Care Attendant.
• Follow physician orders and maintain accurate Plan of Care.
• Document patient visits, progress, and discharge summaries.
• Ensure HIPAA confidentiality and communications with Director of Clinical Services.
