Job summary

Home Care & Hospice in Canyon, Colorado seeks an RN Case Manager to provide 1:1 skilled nursing care in patients’ homes. You will assess, develop and execute a client-centered plan of care with the interdisciplinary team. Schedule is flexible with no weekend requirements and strong clinical manager support.

Must hold Colorado RN license; 1 year nursing experience preferred. Full-time and part-time opportunities available with competitive hourly pay as a Medicare certified agency.

Qualifications

• Graduate of an approved school of professional nursing and currently licensed in Colorado.
• One (1) year nursing experience, preferred.
• Acceptance of philosophy and goals of this Agency.

Responsibilities

• Provides services in accordance with the plan of care.
• Performs initial evaluation visit and develops plan of care with the IDT.
• Reevaluates patient nursing needs and revises care plan as needed.
• Maintains timely clinical notes and summaries of care conferences.
• Coordinates services with physicians, other care providers, patients and caregivers.
• Delivers total patient care as required and adheres to agency policies.
