MaineGeneral Health is seeking a Registered Nurse (RN) Home Health for a nursing role in Augusta, Maine. The position offers 32 or 40 hours per week with limited on-call, weekend, and holiday rotation, and requires travel to surrounding areas within the South Team’s territory.

The role emphasizes patient-centered care, education, and coordination with a multidisciplinary team to manage conditions and optimize outcomes in the home setting.

Qualifications

• Active Maine RN license required.
• At least 1 year of acute or med/surg nursing experience.
• Valid driver’s license with acceptable driving record.
• Experience with electronic medical records preferred.

Responsibilities

• Consult with nursing staff and multidisciplinary team to assess patient status and care appropriateness.
• Provide direct treatment, education, and management of health conditions; make referrals as needed.
• Plan, implement, and evaluate patient care plans to optimize outcomes; monitor and communicate patient condition.
