Job summary

Aspirus At Home in Ironwood, MI is seeking a Registered Nurse Case Manager to join our field team. This role requires managing assessment, planning, coordination and evaluation of nursing care for Home Health patients, with partnerships across Aspirus entities and community partners.

Full-time Day Shift, 1.0 FTE, with on-call weekends; Michigan RN license and BLS required, plus valid driver's license and insurance. Prior home health or hospice experience is beneficial.

Qualifications

• Nursing license current in Michigan.
• BLS certification required or obtained within 90 days of hire.
• Valid driver's license required and must have automobile liability insurance.
• Five years of broad nursing experience; home health or hospice experience beneficial.

Responsibilities

• Assess, plan, implement, coordinate, monitor and evaluate nursing care options for Home Health patients.
• Develop care plans in partnership with Home Health team, Aspirus entities and community partners.
• Coordinate care across the health care continuum and across settings, including on-call weekends.
