Job summary

Help at Home is seeking a Registered Nurse (RN) Case Manager to support LaFayette and Logansport with a $5,000 sign-on bonus. The role includes travel reimbursement and a comprehensive benefits package, including medical, dental, vision, 401(k), unlimited PTO, paid holidays, and wellness programs.

Qualifications

• Graduate of an accredited nursing school.
• One year of clinical nursing experience.
• Active Indiana RN license.
• Experience with medically fragile clients addressing medical, social and behavioral components of care.
• Ability to exercise initiative and independent judgment.
• Ability to work independently and leverage community resources to benefit clients.

Responsibilities

• Provides services in accordance with the plan of care.
• Makes the initial evaluation visit and reevaluates the patient’s nursing needs.
• Initiates the plan of care and revisions.
• Initiates preventive and rehabilitative nursing procedures.
• Prepares clinical and progress notes for each patient visit and summaries of care conferences.
• Coordinates services and informs personnel of changes in patient condition.
• Counsels patient/family in meeting nursing and related needs.
• Performs OASIS assessments and discharge summaries.
• Gives total patient care as needed and participates in on-call duties.
• Goes on weekly travel within assigned territory.
