Job summary

Amedisys is seeking a Registered Nurse case manager to assess patients at home and coordinate care across the team in Pepperell, Dunstable, Tyngsboro, Westford, Groton, Ayer, Littleton, Acton and Concord.

This full-time role offers a $10,000 sign-on bonus and an attractive pay range of $98,100 to $114,000 per year, plus comprehensive benefits such as telehealth medical plans, PTO, HSA, counseling, 401(k), family support, and mileage reimbursement.

Qualifications

• 1+ year of clinical RN experience.
• Current RN license for assigned states.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.
• Note: if <6 months RN experience, participate in RN Intern program.

Responsibilities

• Performs patient assessments and collaborates with care team to develop and implement a plan of care.
• Makes referrals to other disciplines as indicated by patient needs.
• Promotes patient health and independence through teaching and rehabilitative measures.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Communicates patient progress to clinical manager and care team.
• Plans and provides staff education.
• Performs on-call responsibilities and services as assigned.
• Participates in clinical development and continuing education.
