Job summary

Beth Israel Lahey Health is seeking a registered nurse to plan, organize and direct home care services with emphasis on community health education. The nurse builds from community resources to plan and direct services to meet the needs of patients and families within their homes and communities, in accordance with home care regulations and payer guidelines.

Qualifications

• Massachusetts and New Hampshire RN license active and in good standing; willing to obtain the other within 3 months of hire.
• Active American Heart Association BLS certification.
• Associate’s Degree or BSN (preferred) from an accredited school of nursing.
• At least 1 year of direct care nursing experience; OASIS and/or EPIC experience preferred.

Responsibilities

• Complete and document an initial assessment of patient and family to determine home care needs.
• Initiate the plan of care and revise as patient status changes.
• Administer medications and treatments as prescribed by the physician.
• Instruct, supervise, and evaluate the home health aide care every two weeks.
• Identify discharge planning needs as part of the care plan development.
• Maintain confidentiality per HIPAA guidelines.
• Demonstrate proficiency in OASIS completion.
• Document via EPIC in accordance with agency timelines.

What You’ll Do

More specifically:

• Complete and document an initial assessment of patient and family to determine home care needs;
• Initiate the plan of care and make necessary revisions as patient status and needs change;
• Administer medications and treatments as prescribed by the physician;
• Instruct, supervise, and evaluate the home health aide care provided every two (2) weeks;
• Identify discharge planning needs as part of the care plan development;
• Practice confidentiality principles set by the agency and Federal HIPAA guidelines;
• Demonstrate proficiency in OASIS completion;
• Complete documentation through EPIC in accordance with agency timeliness policies.

What You’ll Need

• An active valid Massachusetts and New Hampshire RN license; if currently holding only one state licensure, be willing to obtain the other within 3 months of hire.
• An active American Health Association BLS certification;
• Associate’s Degree or BSN (preferred) from an accredited school of nursing;
• At least 1 year of direct care nursing experience, preferably the area of public health or home care nursing; OASIS and/or EPIC experience is preferred.

Pay Range

$63,200.00 USD - $91,920.00 USD

The pay range listed for this position is the annual base salary range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law.
