Job summary

Interim HealthCare of Oklahoma City is seeking a Home Health RN Case Manager in Oklahoma City. You will join a full care team and shape the plan of care for each patient, going beyond chart notes to truly know your patients.

You’ll participate in a schedule that includes weekdays with some weekend coverage, and you can choose between salary or per-visit pay structures depending on candidacy. Benefits include health coverage and paid time off.

Qualifications

• Active RN license in Oklahoma.
• Home health experience is ideal, but candidates from other clinical backgrounds with strong assessment skills will be considered.
• CPR certification is required.

Responsibilities

• Develop and adjust the plan of care for patients with the care team (LPNs, therapists, physicians).
• Complete visits and OASIS assessments with a comprehensive patient view.
• Coordinate with the care team to prevent gaps in care.

Schedule

• Primarily Monday through Friday, with the flexibility to structure your day around your patients rather than a shift clock
• Full time: two weekends per month, with a weekday off following each weekend worked
• Part time: one weekend per month

What You'll Earn
We offer flexible pay structures for this role, either salary or per-visit, whichever fits you best:

• Per-visit option: $40 per visit, plus $100 for every Start of Care, $75 for every Resumption of Care, and $60 for every Recertification
• Salary option available for the right candidate

What You'll Do

• Play a hands-on role in developing and adjusting the plan of care for your patients, working alongside LPNs, therapists, and physicians
• Complete visits, OASIS assessments, and documentation with an eye toward the full patient picture, not just the task in front of you
• Coordinate with the care team so nothing falls through the cracks
• Educate patients and families on diagnosis, medication, and home safety
• Advocate for your patients and help drive real outcomes

What We're Looking For

• Active RN license in Oklahoma
• Home health experience is ideal, but we're also looking for experienced nurses from any clinical background who are ready for a change and bring strong assessment and critical thinking skills
• CPR certification
• A nurse who wants autonomy, real patient relationships, and a genuine say in the plan of care
