Job summary

UnitedHealth Group is seeking an experienced RN Office‑Based Patient Care Manager to supervise and coordinate home health clinical services. You will lead an interdisciplinary team to ensure physician‑prescribed care is delivered safely in patients’ homes, aligning with state and federal regulations.

Requirements include an active RN license, a valid driver’s license, and strong communication/writing skills. CPR is preferred within 90 days.

Qualifications

• Active RN licensure in state of practice.
• Valid driver's license and access to a dependable vehicle.
• CPR certification preferred within 90 days of employment.
• Ability to work independently.
• Strong communication, writing, and organizational skills.

Responsibilities

• Directly/indirectly supervises home health aides and LPNs, assigns tasks.
• Provides nursing care within scope of practice per plan of care and in coordination with the health care team.
• Performs comprehensive assessments (OASIS), medication reconciliation, and nursing evaluations.
• Manages referrals, clinician assignments, and start of care visits.
• Determines patient home health benefits and ongoing insurance approvals.
• Ensures care aligns with physician orders and eligibility criteria.
• Reviews assessments and plans daily and collaborates with clinicians.
