Job summary

UnitedHealth Group Idaho Home Health is seeking a Registered Nurse in Home Health to provide and direct nursing care to patients in their homes as prescribed by the physician and in compliance with laws, regulations, and agency policies. Qualifications

• Current and unrestricted RN licensure in state of Idaho.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
• CPR Certification preferred or ability to complete within 90 days of hire.
• 1+ years of Home Health experience preferred.
• Ability to work independently, strong communication, writing, and organizational skills.

Responsibilities

• Initiates, develops, implements, and revises the plan of care in collaboration with the physician and other health care professionals.
• Supervises care provided by home health aides and licensed nurses, provides instruction, and assigns tasks according to regulations.
• Provides required supervisory visits and ensures quality, infection control, and timely documentation (OASIS, visits).
• Coordinates total plan of care with physicians, staff, patients, and families to ensure continuity of care.
• Performs timely and accurate documentation of patient visits per policy and payer requirements.
• Participates in on-call and weekend rotation as needed and engages in survey readiness and performance improvement.
