Job summary

UnitedHealth Group’s LHC Group, serving Danbury and surrounding towns, seeks a Registered Nurse to provide and direct home health nursing care under physician orders, in compliance with laws and policies. You will coordinate the overall plan of care with the patient, family, and care team to ensure continuity of service.

Requirements include an unrestricted RN license in the state, CPR certification, and reliable transportation.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current CPR certification.
• Ability to function in home settings with diverse clients.

Responsibilities

• Provide high-quality clinical services within scope of practice and infection control standards.
• Coordinate care with other members of the patient’s care team from admission to discharge.
• Complete clinical nursing assessments per program requirements.
• Ensure patient eligibility and medical necessity for services as defined by payer and policy.
• Develop and revise individualized plans of care with other providers.
• Ensure plan of care frequency and duration meets patient needs and obtain physician approval.
