Concordia Visiting Nurses, a program of Concordia Lutheran Ministries, seeks a compassionate Home Health RN to deliver one-on-one patient care in the home setting. You will conduct assessments, develop individualized care plans with physicians and families, and manage treatments including IV starts and wound care.

Qualifications

• Minimum one year of recent clinical experience as a Registered Nurse recommended.
• Home care experience recommended.
• Adaptability, compassion, and a commitment to patient-centered care.
• Ability to work independently and manage time effectively.

Responsibilities

• Conduct thorough assessments and develop individualized care plans in collaboration with physicians, patients, and families.
• Treatments, IV starts, chest tube management, and wound care.
• Educate and empower patients and families on managing health conditions and building confidence in self-care.
• Utilize strong clinical assessment skills to monitor patient conditions and identify changes.
