Concordia Visiting Nurses is seeking a Home Health Registered Nurse (RN) to provide one-on-one patient care across its Western Pennsylvania service area. You will conduct assessments, develop care plans in collaboration with physicians and families, and deliver skilled nursing services in patients’ homes.

The role emphasizes autonomy, relationship-building with patients and families, and opportunities for ongoing training and professional development within a supportive team.

Qualifications

• Minimum one year of recent clinical experience as an RN recommended.
• Home care experience recommended.
• Adaptability, compassion, and a patient-centered approach.

Responsibilities

• Conduct thorough assessments and develop individualized care plans with physicians, patients, and families.
• Perform treatments, IV starts, wound care, and chest tube management as needed.
• Educate and empower patients and families to manage health conditions and self-care.
