Job summary

Residential Home Health and Hospice is seeking an experienced Registered Nurse to provide skilled nursing in patients' homes in the Bay City, MI area. The role includes a $10,000 first-year success bonus and ongoing rewards for growth.

Qualifications

• Active Registered Nurse with RN license in the state you work.
• One year of RN experience delivering quality patient care.
• Valid driver's license and ability to drive to patient locations (approx. 20%).

Responsibilities

• Deliver skilled, patient-centered care in a home health setting with an interdisciplinary team.
• Perform patient assessments, medication management, and care planning.
• Educate and empower patients and families on disease management and treatment plans.
• Think critically, solve problems in real time, and document thoroughly in the EMR.
• Travel to patient homes within the assigned territory.
• Work independently, but collaborate with the team as needed.
