Job summary

Residential Home Health and Residential Hospice in Bay City, MI is hiring a Home Health Registered Nurse to deliver skilled, patient-centered care in patients’ homes within the Bay City area, with travel to nearby sites as required. You’ll join an interdisciplinary team that supports autonomy and meaningful patient relationships, and you can earn a $10,000 first-year bonus plus ongoing rewards.

Qualifications

• Active Registered Nurse license in the state you work.
• Minimum 1 year RN experience in home health or clinical setting.
• Valid driver's license required due to travel to patient homes.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting.
• Perform patient assessments, medication management, and care planning.
• Educate patients and families on disease management and treatment plans.
• Maintain thorough EMR documentation.
• Work independently with support from an interdisciplinary team.
• Travel to patient homes within assigned territory.
