CenterCare Home Health in Colorado Springs, CO is seeking an experienced RN for Admissions and Case Management. You’ll perform Start of Care, Recertification, and Discharge OASIS assessments and manage a home health caseload with clinical autonomy within a nurse‑led leadership team.

We offer $90,000–$110,000 yearly for full-time roles, bonuses based on productivity, mileage reimbursement, PTO, and health insurance.

Qualifications

• Active RN license in Colorado.
• Home health and OASIS experience preferred.
• Reliable, organized, clinically sound, and patient-centered.
