Job summary

Frontier Home Health in Colorado Springs is seeking an Admissions RN to manage Start of Care visits, comprehensive patient assessments, and care coordination for our growing patient population in the Colorado Springs area.

You will perform OASIS documentation, initiate the plan of care, and collaborate with physicians and the interdisciplinary team. The role emphasizes strong clinical skills, regulatory compliance, and a patient-focused approach to start-of-care transitions.

Qualifications

• Active, unrestricted RN license in Colorado.
• 2+ years RN experience; home health and OASIS experience strongly preferred.
• Current CPR certification required.
• Valid driver’s license and reliable transportation.
• Knowledge of home health regulations, OASIS, and nursing assessment/care planning.

Responsibilities

• Complete Start of Care (SOC) and admission assessments with comprehensive physical, functional, and psychosocial evaluations.
• Develop and implement individualized plans of care per physician orders and Medicare guidelines.
• Perform accurate and timely OASIS assessments and clinical documentation.

Job Summary

The Admissions Nurse (RN) is responsible for completing the start-of-care (SOC) admission visit for patients referred to Frontier Home Health. The Admissions RN conducts comprehensive patient assessments, completes OASIS documentation, initiates the plan of care, and coordinates physician orders and interdisciplinary services.

Essential Duties

• Complete Start of Care (SOC), admission, and resumption of care assessments, including comprehensive physical, functional, and psychosocial evaluations.
• Develop and implement individualized plans of care in accordance with physician orders, patient needs, and Medicare guidelines.
• Perform accurate and timely OASIS assessments and clinical documentation in compliance with regulatory and agency standards.
• Coordinate patient care by collaborating with physicians, therapists, and interdisciplinary team members.
• Communicate changes in patient condition and obtain/verify physician orders as needed.
• Administer skilled nursing care, medications, and treatments as prescribed.
• Identify needs for additional disciplines, durable medical equipment (DME), and community resources.
• Initiate and support discharge planning and care transitions from admission through episode completion.
• Ensure all documentation supports quality outcomes, compliance, and reimbursement requirements.
• Participate in case conferences, care coordination, and quality improvement initiatives (QAPI).

Requirements

• Active, unrestricted Registered Nurse (RN) license in the state of Colorado; graduate of an accredited nursing program (BSN preferred).
• 2+ years of RN experience preferred; home health and OASIS experience strongly preferred.
• Current CPR certification required.
• Valid driver’s license, reliable transportation, and auto insurance.
• Knowledge of home health regulations, OASIS, and nursing assessment/care planning.
• Ability to maintain HIPAA compliance and adhere to all regulatory and agency standards.
• Completion of required TB screening and health documentation.
