Job summary

Phoenix Home Care and Hospice in Kansas City, MO is seeking an RN to provide skilled, in-home nursing visits on a full-time, on-call overnights & weekends schedule (7 on / 7 off). You will deliver compassionate patient care, wound management, education on health conditions, and collaborate with Physical, Occupational and Speech Therapists to support recovery and comfort.

A valid MO RN license and reliable transportation are required.

Qualifications

• Active RN Nursing License in Missouri
• Ability to work in a fast-paced, dynamic environment
• Excellent organizational and self-management skills
• Strong and compassionate customer service skills
• Valid driver’s license and reliable transportation

Responsibilities

• Skilled visits to patients based on their needs
• Daily patient interaction, ensuring comfort and support
• Education on health conditions including CHF, COPD, Diabetes
• Wound care and assessments
• Medication management and administration
• Change dressings, bandages, and other contraptions as prescribed
• Documentation of patient's condition and reporting changes to physician
• Collaboration with Physical, Speech, and Occupational Therapists

Job Duties

• Skilled Visits to patients based on their needs.
• Daily patient interaction, ensuring comfort and support.
• Education on health conditions including, but not limited to, CHF, COPD, Diabetes.

• Medication management and administration.
• Change dressings, bandages, and other contraptions as prescribed.

• Collaboration with other healthcare professionals, including Physical, Speech, and Occupational Therapists.
