Job summary

Phoenix Home Care & Hospice in Missouri seeks an experienced RN to provide skilled nursing visits in Eldon, Versailles, and surrounding areas. You will assess patients, implement physician orders, and collaborate with a multidisciplinary team to build individualized care plans.

Qualifications

• Current Registered Nurse or Licensed Practical Nurse license in Missouri.
• Home Health or Critical Care experience preferred.
• Driver's license and vehicle with current auto insurance.
• Ability to Pass Drug Test, Background Check, and Physical.

Responsibilities

• Assists in development, review and revision of the plan of care
• Provides nursing care in accordance with physician's plan of treatment and client's plan of care
• Observes, records and reports reactions to treatment and changes in client's condition
• Provides ongoing instruction and supervision of client, family or health team personnel
• Participates in education programs, quality assessment activities and staff meetings
• Monitor and record patient's condition and document care via electronic charting
