Halcyon Home is seeking a dedicated and compassionate Home Health Registered Nurse Full Time to support our growing geriatric home health population in San Marcos, TX.

Qualifications

• Current RN license in good standing with the state nursing board.
• Experience in home health or related healthcare setting preferred.
• Strong clinical and assessment skills.
• Excellent communication and interpersonal abilities.
• Ability to work independently and manage time effectively.

Responsibilities

• Initial and ongoing assessments of patient needs across clinical domains.
• Apply admission criteria for hospice care and determine level of care.
• Develop and document the plan of care to ensure quality and continuity of care.
• Communicate effectively with patient, family, team members, and other healthcare professionals.
• Follow infection control and standard precautions in patient care.
• Provide clinical directions to Hospice Aide and LPN/LVN to ensure quality and continuity of service.
• Prepare progress notes that document goal progress and patient outcomes.
• Inform physician and others of changes in patient needs and response to care.
• Monitor assigned cases for third-party payor compliance.
• Demonstrate commitment, professional growth and competency.
• Educate patient and family regarding disease, self-care, end-of-life care; prioritize needs with IDG; train staff as needed.
• Determine scope and frequency of services based on acuity and family needs.
