Graham Healthcare Group is seeking a Registered Nurse (RN) Home Health to provide skilled nursing care to patients in Prospect Heights, IL and surrounding areas. The role includes travel to patient homes, a 40-hour work week, and an opportunity to earn a first-year $10,000 bonus based on performance.

The RN will perform assessments, manage medications, educate families, and document in EMR, collaborating with an interdisciplinary team and leadership for growth and support.

Qualifications

• Active RN license in the state you work.
• One year of RN experience in home health or similar.
• Valid driver's license and ability to travel to patient homes.

Responsibilities

• Deliver skilled, patient-centered home health care in patients homes.
• Perform patient assessments, medication management and care planning.
• Educate patients and families on disease management and treatment plans.
• Document thoroughly in EMR and maintain compliance.
• Work independently within an interdisciplinary team and travel within-territory.
• Support autonomous practice while staying connected to leadership.
