Job summary

Adoration Health in Crestview, FL is seeking a dedicated Home Health Registered Nurse to provide 1:1 patient care in the home setting. You will assess needs, develop and implement care plans, administer medications, and support families while working with a collaborative team.

Requirements include an active RN license, 1 year of nursing experience (home health/geriatric preferred), CPR, and a valid driver's license. A sign-on bonus may apply for external candidates.

Qualifications

• RN with current license in state of practice and residence, required.
• One year of nursing practice in patient care; home health or geriatrics preferred.
• Valid driver's license, driving record and insurance per policy.
• New graduates may be considered in select markets.

Responsibilities

• Assess and monitor physical, emotional, and psych needs of patients.
• Create home health care plans aligned with MD orders and patient goals.
• Administer medications, treatments, and interventions.
• Provide pain and symptom management and educate families.
• Collaborate with interdisciplinary team and maintain timely documentation.
• Participate in on-call rotation as required.
