Job summary

BrightSpring Health Services Corporation is seeking a Registered Nurse for home health in Brunswick, GA. The Home Health RN will provide one-on-one patient care, develop care plans, administer treatments, and collaborate with a multidisciplinary team to support patient goals in a community setting.

Ideal candidates have an RN license, at least 1 year of nursing experience (home health or geriatrics preferred), and CPR certification.

Qualifications

• Nursing degree with current unrestricted license in state of practice.
• Minimum 1 year of nursing experience; home health/geriatrics preferred.
• Valid driver's license and proof of car insurance.
• New grads may be considered in select markets.
• Current CPR certification.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create home health care plans that align with MD orders and patient goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient's family and caregivers.
• Collaborate with an interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by the local branch.
