Qualifications
- RN license in state of practice and residence
- 1 year nursing experience; home health or geriatrics preferred
- Valid driver's license with proof of insurance
- Current CPR certification
Responsibilities
- Assess/monitor physical, emotional, and psychological needs of patients
- Create home health care plans per MD orders and patient goals
- Direct nursing care: medications, treatments, and interventions
- Provide pain and symptom management
- Educate and support patient families and caregivers
- Collaborate with an interdisciplinary team
- Maintain accurate and timely documentation
- Participate in on-call rotation as required by the local branch
