Job summary

Adoration Health is seeking a dedicated Home Health Registered Nurse to join our Fayetteville, WV team. This role focuses on one-on-one patient care, developing individualized home health plans, and coordinating with MD orders to meet patient goals.

The RN will administer medications, educate families, and work with an interdisciplinary team to ensure quality outcomes. A valid RN license and CPR certification are required, with one year of nursing experience preferred.

Qualifications

• Registered Nurse degree (associate or bachelor) with current unrestricted license in state of practice and residence.
• CPR certification is required.
• One year nursing practice in a patient care setting; home health, geriatrics or related settings preferred.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create home health care plans aligned with MD orders and patient goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient's family and caregivers.
• Collaborate with an interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by the local branch.

Education

Registered Nursing Degree (Associate or Bachelor) from an accredited college of nursing; current unrestricted registration and license in the state of practice and in the state of residence
