Job summary

Elara Caring in Ashtabula County is seeking a Registered Nurse Home Health to join our team. You will deliver high-quality, compassionate care to patients in their homes, coordinating with the care team to support patients on their health journey.

This role offers autonomy with flexible schedules, 1:1 patient care, and competitive compensation. Travel within the branch area is required; valid driver's license and vehicle are necessary.

Qualifications

• Current, unrestricted state RN licensee.
• 1+ year experience in a clinical care setting as a nurse.
• Able and willing to travel within branch/office coverage area.
• Ability to frequently lift, push, pull, and support up to 50 pounds.
• Must have a dependable vehicle, valid driver’s license, and current auto insurance.
• Must be able and willing to travel 50%.
• Associates Degree or Bachelor’s Degree in Nursing preferred.
• Home Health or Hospice experience preferred.

Responsibilities

• Ensures activities align with Elara Caring’s vision and leadership.
• Maintains patient and staff privacy and confidentiality per HIPAA.
• Assesses caseload to identify physical, psychosocial, and environmental needs.
• Assumes primary responsibility for caseload planning and evaluation.
• Assesses patient needs and data to inform the plan of care.
• Makes initial evaluation and follow-up visits as needed.
• Communicates findings and changes to Clinical Supervisor/physician.
• Revises the plan of care and notifies team members.
