Job summary

Sweetwater Hospital Association is seeking a full-time Home Health RN to join our healthcare team in Tennessee. The RN will provide compassionate, quality nursing care to patients in their homes, promoting independence, safety, and well-being.

Responsibilities include assessing conditions, administering medications, educating patients and families, documenting care, and coordinating with physicians and the healthcare team.

Qualifications

• Graduate from an accredited nursing program with a current Tennessee license.
• Experience is preferred; orientation provided.

• Provide skilled nursing care to patients in their homes in accordance with established standards and individualized care plans
• Assess patient conditions and monitor changes in health status

• Communicate effectively with physicians, patients, families, and other members of the healthcare team
• Accurately document patient assessments, care provided, and changes in condition
• Coordinate patient care and follow established plans of care
• Complete required clinical and administrative documentation

• Maintain compliance with hospital policies, procedures, and applicable state and federal regulations

Education:

• Graduate from an accredited school of nursing with a current license in the State of Tennessee.

Experience:

Experience is preferred; however, all employees will go through department‑specific orientation.
