Job summary

Residential Home Health and Hospice is seeking a Registered Nurse to provide skilled nursing care to patients in their homes across Jackson County, MO. This position offers autonomy, collaboration with an interdisciplinary team, and opportunities to grow.

You will perform patient assessments, meds management, care planning, and educate families on disease management. A $20,000 total bonus potential, including a $10,000 first-year success bonus and a $10,000 sign-on, awaits the right candidate.

Qualifications

• Active RN license in the state you work
• Minimum one year of RN experience delivering skilled nursing care
• Valid driver's license and ability to travel ~20% of workday to patient homes

This position is eligible for a $20,000 total bonus potential, including a $10,000 first year success bonus to reward the accomplishment of key achievements during your first year as well as an additional $10,000 sign-on bonus paid within the first 18 months of employment! What You’ll Do:

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Perform patient assessments, medication management, and care planning
• Educate and empower patients and families on disease management and treatment plans so they feel confident, informed, and cared for
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory

RN Qualifications

• Active Registered Nurse, RN license in the state you work
• One year of RN experience and the clinical expertise to deliver quality patient care
• Due to the field-based nature of this home health position, a valid driver's license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
