Join our team at Valentine Recruiting as a Home Health Registered Nurse, RN. We are seeking a dedicated and compassionate nurse to provide high-quality patient care in a home health setting. This role involves assessing patient needs, developing care plans, administering medications, and collaborating with healthcare professionals to ensure comprehensive care.

Responsibilities

• Provide direct patient care across various age groups and health conditions.
• Conduct assessments and document patient histories accurately.
• Develop and implement individualized care plans.
• Administer medications and treatments as prescribed.
• Monitor and record vital signs and patient conditions.
• Collaborate with interdisciplinary teams for optimal outcomes.
• Educate patients and families on health management and disease prevention.
• Maintain accurate patient records and documentation.

Requirements

• Minimum of one year recent experience in an acute care setting.
• Active RN license in California.
• Per Diem applicants: at least 6 months of home health experience as an RN.
• Full-Time applicants: home health experience not required.
• Graduate of an accredited nursing school; BSN or PHN preferred.
• Valid driver’s license, insured vehicle, and good driving record.
• Current CPR certification.
• Strong communication and interpersonal skills.
