Shift: 5x8s – Days with required weekend, on-call, and floating

Job Description:
Sunbelt Staffing is hiring a Travel Home Health RN for an exciting 13-week contract in Trumbull, CT. This role allows you to bring your clinical expertise into patients’ homes while exploring a beautiful and welcoming New England community. Locals and travelers both accepted.

Key Responsibilities:

Manage a caseload of home health patients

Perform SOC, ROC, Recert, and Discharge visits

Document all patient care activities using Homecare Homebase (HCHB)

Educate patients and caregivers on disease processes and home care procedures

Participate in weekend, on-call, and float rotations as needed

Qualifications:

Current CT RN license

At least 2 years of recent home health nursing experience

Proficiency with Homecare Homebase (HCHB)

Current AHA BLS certification

Must have reliable transportation, valid driver’s license, and current car insurance

Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission
