Summary of Essential Functions for the Position:

Committed to Caring: A Registered Nurse Case Manager (RN) administers skilled nursing care to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and the plan of care under the supervision of the Executive Director and Director of Clinical Services. The RN participates in patient, caregiver, and family counseling and education, communicates with physicians and other healthcare practitioners involved in the plan of care, and provides services ordered by the physician. The RN also contributes to the QAPI program, participates in interdisciplinary assessments, and collaborates in developing and revising the plan of care with patients and caregivers. Supervises care provided by LVNs and HHAs.

Qualifications

• Licensed in the practicing state(s)
• At least 2 years of experience, preferably in Home Health
• CPR certification from the American Heart Association or American Red Cross
• Ability to exercise initiative and independent judgment
• Strong communication and interpersonal skills
• Good customer relations and commitment to quality service
• Valid driver's license, auto insurance, reliable transportation
• On-call at least once per month
• May require 100% travel

Educational Requirements

• High School Diploma or GED; graduate of an approved nursing school

Working Conditions & Physical Requirements

• Work indoors at the agency and in patient homes, with travel involved
• Requires physical effort including lifting, reaching, and exposure to environmental elements
• Must provide evidence of TB testing and other required examinations

Shift

• Monday-Friday, 8:30 AM - 5:00 PM

Coverage Area

• Southeast Houston: La Marque, Pasadena to Galveston
