Job summary

The Roads Home Health Care in South Florida is seeking a dedicated Home Health Registered Nurse (RN) to provide skilled nursing care to patients across the region. You will work with a Case Manager to plan, coordinate, and document care for an assigned caseload, delivering hands-on patient care within a supportive clinical team.

The role requires Florida RN licensure, CPR certification, and at least one year of acute care nursing experience; home health experience is preferred.

Qualifications

• Active RN license in Florida.
• Current BCLS/CPR certification.
• Minimum one year of acute care nursing experience.
• Prior home health experience preferred.
• Valid Florida driver's license and reliable transportation.

Responsibilities

• Provide skilled nursing care in patients' homes per plan of care.
• Assess and update the plan of care as needed.
• Assess and manage patients' pain and educate patients and families.
• Administer medications per patient status and best practices.
• Obtain physician orders and report changes promptly.
• Participate in case conferences to discuss patient progress and coordinate multidisciplinary care.
• Teach, supervise, and counsel patients and family members on nursing care needs.
• Complete accurate documentation and charting for every visit.

About the Role

The Roads Home Health Care is looking for a dedicated Home Health Registered Nurse (RN) to provide skilled nursing care to patients across South Florida. Working alongside a Case Manager, you will help plan, coordinate, and document care for an assigned caseload, delivering hands-on patient care that meets each individual's needs. • Provide skilled nursing care in patients' homes, following the individualized plan of care
• Assess each patient's condition, updating and adjusting the plan of care as needed
• Assess and manage patients' pain, using appropriate techniques and educating patients and families
• Administer medications correctly based on patient age and clinical status, following best practices to prevent errors
• Obtain and follow physician orders, reporting changes in a patient's physical or emotional condition promptly
• Participate in case conferences to discuss patient progress and coordinate multidisciplinary care
• Teach, supervise, and counsel patients and family members on nursing care needs
• Complete accurate, timely documentation and charting for every visit

Job Requirements

• Active Registered Nurse (RN) license in the State of Florida
• Current BCLS/CPR certification
• Minimum one year of acute care nursing experience
• Prior home health experience preferred
• Valid Florida driver's license, current automobile insurance, and reliable transportation

Qualifications

• Comfortable delivering patient care across the lifespan, from pediatric to geriatric patients
• Strong verbal and written communication skills for coordinating with physicians, patients, and families
• Basic computer proficiency for entering and managing patient documentation
• Working knowledge of federal, state, and agency regulations governing home health care
• Respectful, culturally sensitive approach to patient and family education
• Bilingual candidates are encouraged to apply
• Experience with home health EMR or documentation software is a plus
