Havenova Home Care Solutions is seeking a Registered Nurse (RN) to provide clinical oversight, care planning, and Medicare compliance across our Massachusetts home care operations. The role emphasizes high-quality patient care, accurate documentation, and regulatory adherence.

You will collaborate with caregivers, families, and healthcare professionals to coordinate care, supervise chart audits, and guide staff to meet care plans and regulatory standards.

Qualifications

• Active RN license in MA.
• Home care, home health, or case management experience preferred.
• Strong knowledge of clinical documentation and compliance standards.
• Experience with Medicare guidelines preferred.

Responsibilities

• Conduct patient assessments and develop individualized care plans
• Review and maintain accurate clinical documentation in compliance with regulatory standards
• Coordinate care with caregivers, physicians, case managers, and families
• Monitor patient condition and document changes appropriately
• Ensure care delivery aligns with care plans and clinical needs

The RN will work closely with caregivers, families, and healthcare professionals to coordinate care and maintain compliance with all applicable clinical and documentation requirements.

Qualifications

• Excellent communication and interpersonal skills
• Strong attention to detail and organizational skills
• Valid driver’s license and reliable transportation
• CPR certification required
