TEEMA is seeking a Home Health Registered Nurse to deliver personalized nursing care in patients’ homes across Oregon. You will assess patients, create and adjust care plans, and educate families on disease management and recovery.
Collaboration with physicians and a multidisciplinary team ensures high-quality outcomes. Schedule is Monday–Friday, 8:00 AM–4:30 PM with rotating weekend coverage among clinicians; maximum 40 hours per week.

As a Home Health RN, you will provide personalized nursing care, helping patients recover from illness, manage chronic conditions, and maintain independence in the comfort of their own environment.
Provide skilled nursing care to patients in their homes, including private residences, assisted living, and adult foster homes
Perform comprehensive patient assessments and develop individualized care plans
Monitor patient progress and adjust care plans to support optimal outcomes
Educate patients and families on disease management, medications, and recovery
Collaborate with physicians and interdisciplinary teams to coordinate care
Maintain accurate, timely documentation of assessments, treatments, and outcomes
Advocate for patients and support their safety, comfort, and well-being
BLS certification
Minimum 1 year RN experience in acute care
Valid Oregon driver’s license, reliable vehicle, and current auto insurance
Home Health, Community Health, or Critical Care experience

Rotating weekend coverage shared among clinicians
