Job summary

Interim HealthCare of Augusta is seeking a Home Health Registered Nurse (RN) to provide one-on-one care to patients in Danville, VA and surrounding areas. This role offers flexible scheduling (FT, PT & PRN) and travel to patient homes across the service area.

You will evaluate patients, develop care plans, educate families, and coordinate with physicians and the care team to ensure quality outcomes in a home setting.

Qualifications

• Graduate of an accredited nursing program.
• Active RN license in Virginia.
• Minimum of 1 year of nursing experience (home health preferred).
• Current CPR certification and TB screening.
• Valid driver’s license, reliable transportation, and auto insurance.
• Willingness to travel throughout the service area.
• Strong clinical judgment, communication, and organizational skills.
• Compassionate and patient-focused approach to care.

Responsibilities

• Provide skilled nursing care to patients in a home setting.
• Complete patient assessments and monitor patient progress.
• Develop, implement, and update individualized care plans.
• Educate patients and families on medications, treatment plans, diet, exercise, and home safety.
• Coordinate care with physicians, LPNs, CNAs, and other healthcare team members.
• Maintain accurate and timely documentation.
• Identify changes in patient condition and communicate updates to physicians and care team.
• Coordinate patient discharge planning and ensure care goals are met.

Job Overview:

We are currently seeking a compassionate and skilled Home Health Registered Nurse – RN to provide personalized, one-on-one patient care throughout Danville, VA and the surrounding areas. This position is ideal for an RN seeking flexibility, autonomy, and the opportunity to make a meaningful impact while caring for patients in the comfort of their homes.

As a Home Health Registered Nurse – RN, you will provide skilled nursing care to patients recovering from illness, injury, or chronic conditions while collaborating closely with physicians, families, and interdisciplinary care teams. This role requires travel to patient homes throughout the service area.
