Job summary

Illinois Home Health, part of LHC Group and the Optum family, is seeking a Registered Nurse to provide and direct home-based nursing care as prescribed by physicians and in accordance with laws and agency policies.

You will coordinate the total plan of care with other health professionals, conduct supervisory visits, and document care and assessments (OASIS) while maintaining high-quality standards and patient safety in the home setting.

Qualifications

• Current and unrestricted RN licensure in state of practice
• Current CPR Certification or ability to complete within 90 days of hire
• 1+ years of Home Health experience
• Solid communication, writing, and organizational skills

Responsibilities

• Initiates, develops, implements, and revises the plan of care with the physician and health professionals
• Supervises care provided by home health aides and licensed practical/vocational nurses, provides instruction, and assigns tasks according to State and federal regulations
• Provides required supervisory visits
• Completes comprehensive assessments (OASIS) including medication reconciliation accurately and timely
• Documents patient visits per policy and payer requirements
