Job summary

VNA of Maryland, part of the LHC Group family, seeks a Registered Nurse in Home Health to deliver and coordinate nursing care for patients in their homes, aligning with physician orders and agency policies. The role emphasizes collaboration with physicians, families, and the health care team to ensure continuity of care.

The RN will supervise home health aides and other staff, complete OASIS assessments, and maintain compliance with regulations while supporting patient outcomes and quality care.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.

Responsibilities

• Initiates, develops, implements, and revises the plan of care in collaboration with the physician and other health care professionals.
• Supervises care provided by home health aides and licensed practical/vocational nurses, provides instruction, and assigns tasks according to State and federal regulations.
• Provides required supervisory visits.
• Provides high-quality clinical services within the plan of care and infection control standards; completes comprehensive assessments (OASIS) and documents visits per policy.
• Communicates information timely with physicians, patients, and family members to ensure quality care and service excellence.
• Participates in survey/readiness activities and performance improvement plans; participates in on-call and weekend rotation.
